Medicare Part B and Advantage PPO:

Billed to Medicare. No additional fee other than copay.

Initial Consultation Fee (non-Medicare):

$250 for comprehensive consultation including wellness labs
$150 for follow up 

Membership Fee (non-Medicare): 

$65 per month per adult age 19-50 years
$85 per month for adult age 51 and above without Medicare
$120 per month for couples
$250 for couples with 2 adult children
Small Business Plan (5 + employees) : call for plans
